LBH Care Quality Program

In order for Lukas Behavioral Health LLC to be able to effectively and fairly investigate complaints or concerns, individuals must submit all complaints, concerns, or feedback to Quality Assurance Team.

Each complaint will be investigated fully, and in doing so Quality Assurance may reach out to the involved individuals to obtain additional information.

Once you have submitted a complaint, our quality assurance team will review the complaint, typically within 3-5 business days; at which point you will receive an email notifying you of receipt and investigation.

Upon the conclusion of the investigation, the Quality Assurance Team will provide the individual(s) who filed the complaint with a final report which will include the outcome of the investigation.

Individuals filing complains should rest assured that this process is judgment free and there should be no fear of any kind of retaliatory actions.

Who can file a complaint?

Patients or their family members, as well as staff of LBH may file a complaint for any issue that causes concern.
